About Us

Peninsula Home Hospice is your community-based palliative care service.

Our aim is to provide specialist palliative care in the home that optimises quality of life and honours the hope for comfort, choice, dignity and peace.

There are many in our community who are living every day with a life threatening illness and who need support to be able to live their lives as normally as they can.

Dying, as with living, should include choices such as being supported to stay in one's own home surrounded by family and friends, but many of us don't have a big enough family or sufficient friends to give us the care we need. PHH's palliative care is there to help.

Our service is provided to all residents within the Mornington Peninsula Shire, City of Frankston and parts of the City of Kingston (south of Mordialloc Creek).

Services are provided free of charge.

What We Do

Because palliative care is based on individual needs, the services offered will differ but may include:

  • Relief of pain and other symptoms
  • Resources such as equipment needed to aid care at home
  • Assistance for families to come together to talk about sensitive issues
  • Links to other services such as home help and financial support
  • Support for people to meet cultural obligations
  • Support for emotional, social and spiritual concerns
  • Counselling and grief support
  • Referrals to respite care services
  • Bereavement support is also offered to family and friends involved in providing care
