Country Health SA has a vision to  be the best rural health service. Growing better services in country & keeping people well at home.

Country Health SA is part of SA Health & oversees the rural public health system in South Australia. We are transforming health care & delivering health benefits so that South Australians living in rural & remote areas lead healthier lives.

We provide acute health services to over 100,000 people & a further 175,000 people annually across country emergency departments. We have a network of 64 hospitals & approximately 220 health services sites, & employ over 8000 people.

Country Health Connect offers a wide range of community, in-home & residential aged care, early childhood & disability services across rural & regional South Australia, including support for Aboriginal & Torres Strait Islander communities & those from diverse cultural backgrounds. How do I access Country Health Connect services?
To access Country Health Connect services for you or your loved ones, we must first determine if you/they are eligible. Simply contact us on 1800 944 912 to discuss your needs. After the initial discussion with one of our team members, we will determine the best program to meet your needs, which may require an assessment by the My Aged Care team. If so, we will request a representative from My Aged Care to contact you to start the process.
What is Patient Assistance Transport Scheme (PATS).
The Patient Assistance Transport Scheme (PATS) is a subsidy program funded by the Government of South Australia to assist people travelling more than 100km each way to receive approved medical specialist's services. The scheme is available to eligible South Australians residents & approved escorts who do not have the access to medical specialist services locally. For more information on PATS, visit our website,
Am I eligible for Patient Assistance Transport Scheme (PATS)?
To be eligible for Patient Assistance Transport Scheme (PATS), subsidies, clients need to meet the following criteria: be a permanent South Australian resident, travel more than 100km one way to seek approved medical specialist services that is not available locally, receive treatment claimable under Medicare from an approved medical specialist, have claimed any available benefits from a private health fund first, if applicable. 'Telehealth' or 'Telerehabilitation' uses technology to deliver real-time health services to you in your home or local health clinic. Privacy & confidentiality standards will be the same as face-to-face sessions. For more information, visit our website

What are the benefits of telerehabilitation?
There are number of benefits to using the telerehabilitation services. A mix of telerehabilitation & face-to-face sessions can reduce travel time, accommodation & parking costs, waiting time, & stress on you, your family members & carers.
How does the telerehabilitation process work?
Using the telerehabilitation services is easy & simple to use. You can use your own iPad, tablet or computer to access the services. Your health care professional can help you setup the service on your device. You may be provided with a loan iPad to take home to access the services. Your health care professional will show you how you use the device.
