How do I access Country Health Connect services?
To access Country Health Connect services for you or your loved ones, we must first determine if you/they are eligible. Simply contact us on 1800 944 912 to discuss your needs. After the initial discussion with one of our team members, we will determine the best program to meet your needs, which may require an assessment by the My Aged Care team. If so, we will request a representative from My Aged Care to contact you to start the process.
What is Patient Assistance Transport Scheme (PATS).
The Patient Assistance Transport Scheme (PATS) is a subsidy program funded by the Government of South Australia to assist people travelling more than 100km each way to receive approved medical specialist's services. The scheme is available to eligible South Australians residents & approved escorts who do not have the access to medical specialist services locally. Am I eligible for Patient Assistance Transport Scheme (PATS)?
To be eligible for Patient Assistance Transport Scheme (PATS), subsidies, clients need to meet the following criteria: be a permanent South Australian resident, travel more than 100km one way to seek approved medical specialist services that is not available locally, receive treatment claimable under Medicare from an approved medical specialist, have claimed any available benefits from a private health fund first, if applicable. Your health care professional can help you setup the service on your device. You may be provided with a loan iPad to take home to access the services. Your health care professional will show you how you use the device.
